: : If there is a tie for 1st or 2nd, they usually send you to playoffs as T1 or T2. If you are tied for 3rd (in a 5 or 6 team league) they need to clarify who is 3rd and who is 4th, as 4th won't make the playoffs automatically.

: : How they determine that is league to league. I think most leagues do have that point system that the previous post mentioned, but they usually doesn't settle the tie. Some leagues will do a simple coin flip. Some leagues will have a play in game between the two teams that are tied for 3rd on Friday.

It all depends on the league by laws. I have been in leagues where I had to sit through a three way coin toss to determine second through fourth. I have been in other leagues that send teams in as tied for first or whatever. Some leagues have a playoff. It just depends on the league bylaws.
